Regular Open Registration: May 1 to May 29

Lat Registration (all students): May 30 to beginning of school year

New Student Download Forms

We are excited to provide you with all the information you need to enroll your child in our school. As a new student, there are a few important forms that you will need to fill out before your child can begin attending our classes.

Please note the following forms are for new students only:

1. Authorization Form: This form authorizes our school to provide medical treatment to your child in case of an emergency. It is important that we have this form on file so that we can provide the best possible care for your child.

3. New Student Needs Checklist Form: There are several documents and forms you will need to provide to the school to complete your enrollment. Here is a checklist of some of the most common documents required.

4. Physical Exam Form: This form verifies that your child has had a recent physical examination and is in good health. It is important that we have this information on file so that we can ensure your child is healthy and able to participate in all school activities.
